Enterprise Health: U-M Vaccination History Form

U-M students living in campus housing are required to complete the U-M Vaccination History form. Information regarding this policy is available on the Michigan Housing Vaccination Requirements Frequently Asked Questions page.

Before accessing the form, it is highly recommended that you gather all of your vaccine information and documentation. You will need:

  • Type of vaccines, including brand names, if available.
  • Dates received.
  • Supporting documentation; this must be submitted as a single file (pdf, .jpg or .png).

The name on your vaccination records needs to match your name as displayed in Wolverine Access. If it does not, update your preferred name using the Student Self Service tile in Wolverine Access. If your legal name is incorrect, contact the Registrar’s Office. 

Accepted sources of vaccination records are:

  • MCIR record (preferred source for students from Michigan)
  • State-based Immunization Information Systems
  • County-based Immunization Systems
  • Health system portal
  • Pediatrician's office
  • Pharmacy records
  • Vaccination card
  • Country or Ministry of Health records

The U-M Vaccination History form is accessed from Vax Viewer. Go to Wolverine Access > Health Response & COVID 19 tile > Vax Viewer.

Step-by-Step Instructions

  1. Review the information in the blue box at the top of the Vax Viewer page.
  2. Click the Student Portal link in the blue box.
  3. Click Message Center or the New Notifications or unread messages icon (white bell).
  4. Click begin on the U-M Vaccination History form row.
    • If you had previously accessed this form and saved your progress, click resume.
  5. Review the Introduction information.
  6. Review the Consent information.
  7. (required) Click I consent to agree to the collection of vaccine information as described in the Consent section.
    • Consent is required. The form cannot be submitted unless consent is given.
  8. Enter all vaccine information matching your supporting documentation:
    • For each vaccine, select the vaccine type from the drop-down list.
      • If you select Other, enter the vaccine type in the box that displays.
    • Enter the date(s) for each vaccine dose.
      • Read each date description carefully. Some vaccines require multiple dates, and others require the most recent dose date only.
  9. (required) Click the applicable button to answer the Have you COMPLETELY finished entering your vaccination history above? question.
    • Click Yes if you have finished entering all of your vaccine information or have no vaccines to enter. Go to step 10.
    • Click No if you have not finished entering your vaccines, then click Save for Later. You can close the form.
      • When you are ready to finish entering your vaccine information, go to step 1.
      • Do not upload any vaccine documentation until you have entered all vaccination information.
  10. Click either Yes or No to the Did you enter vaccine information above? question.
    • If you click Yes, click the Choose File icon, then navigate to and select the single file that contains your proof of vaccine(s).
  11. Click Submit.
    • If you are not ready to submit the form, click Save for later.
    • The status of your vaccine information can be checked on Vax Viewer. It is updated at 6:30 a.m., Monday - Saturday.
    • If your form is rejected, you will receive an email with further instructions
